Registration Information Conference Hotel Information Adam’s Mark Hotel Our host hotel is the Adam's Mark Hotel , 111 Pecan Street East . Located on the River Walk at Soledad Plaza in downtown San Antonio , the hotel is within walking distance of the Alamo, Convention Center, and Hemisfair Plaza. A special room rate of $108 single/double is guaranteed for reservations made by June 20, 2004 or until the room block is filled. Reservation requests after this date will be accepted on a space-available and rate-available basis. Make your reservation today by calling the number above. Please identify yourself as a participant in the 2004 Summer Institute on Evidence-Based Practice. Book early to assure a room at the hotel. Registration Information Please complete the registration form in this brochure. You can register: 1. By FAX*: Fax the completed registration form with credit card information (VISA, Mastercard, or Discover) to Continuing Medical Education, Fax (210) 567-6964. 2. By MAIL*: Send check or money order (payable to UTHSCSA-CME) with the registration form to: The University of Texas Health Science Center at San Antonio, Continuing Medical Education Department, 7703 Floyd Curl Drive, MSC 7980, San Antonio, TX 78229-3900. *Registrations must be postmarked or faxed by June 2, 2004 to qualify for the Early Bird Fee of $450, or by June 30, 2004 to qualify for the regular fee of $475. Inquire about group rates and policy at acestar@uthscsa.edu . Registrations received after June 30, 2004 or on site must pay $525. Be sure to indicate your preference for the concurrent sessions you with to attend. This will help us to provide appropriate seating during the Institute. You will NOT be assigned a session if you do not indicate one. Registration Confirmation Written confirmation will be sent by mail if your registration is received by June 30, 2004 . If you are registering less than 2 weeks before the program, or if you do not receive a confirmation letter, call the Continuing Medical Education office at The University of Texas Health Science Center at San Antonio (210) 567-4446 to confirm space availability and to receive priority for on-site registration if space is available. You are responsible for ensuring that your registration has been received. Refunds and Camcellation Policy UTHSCSA-Continuing Medical Education reserves the right to limit enrollment or to cancel any course no less than one week prior to the activity. Should circumstances make this necessary, only registered attendees will be informed. Tuition would be refunded in full. You are encouraged to send a substitute if you are unable to attend, once registered. The cancellation must be received in writing and postmarked or faxed no later than June 30, 2004 . Cancellations cannot be accepted over the telephone. The registration fee will be refunded, minus a $50 administrative fee if notice is received by June 30, 2004 . After June 30, 2004 there will be no refund of registration fees. The conference hosts reserve the right to substitute speakers or cancel and reschedule sessions due to any unforeseen circumstances.
